Epicenter Activity Request
Submit only activities that will take place in the Epicenter Building. This form goes directly to Leighanne Guthrie. Please fill out completely.

EPICENTER FACILITY USE POLICY
Individuals and Groups who may use the facilities:
- Epicenter youth, programs & sponsored groups
- Clarksville First Church of the Nazarene members/attendees
- Non-profit Groups
Guidelines for use of the building:
There is no smoking anywhere in the building or within 30 feet of the entrance doors.
Alcoholic beverages or illegal drugs are not allowed on church property.
All those using the facilities are asked to show respect for all property, and are expected to leave rooms and spaces as they were found. This includes removing all trash from the area used and placing it in the outside dumpster, returning any supplies to designated areas, cleaning up all used spaces.
Application & Fee $50.00 per event
Complete the following application and submit it to the church office. The office administrator will arrange for a Special Events Coordinator from the Epicenter to be assigned to the event. The Epicenter does not charge a fee for using the facilities, but does charge $50.00 per event for a Coordinator to be present during the event. Once the event and date has been approved, and fee has been collected, you will be contacted with the approval.
Coordinator
The Epicenter staff will maintain a list of qualified coordinators willing to serve events held at the Epicenter. There must be a coordinator at each event held at the Epicenter. The coordinator will be present at all times during the event for assistance and coordination purposes, to ensure proper use of the equipment and utilities, including sound/lighting system & concession stand and to ensure security of the facility. The coordinator is the only person allowed to use the sound/lighting system and is to be the only one running the concession stand, if needed. Coordinator will assist the group with the use of the TV's, DVD/VCR, & Gaming Systems.
Liability Insurance
Any organization using the Epicenter facilities should provide its own liability insurance against possible claims of injury or damage while on Clarksville First Church of the Nazarene property.
General Guidelines
The individual or group authorized to use the facility is responsible for the following:
- All set up and tear down of the tables and chairs (if needed)
- Sweeping and/or mopping spills
- Cleaning counters and other equipment used
- Washing Tables
- Removal of all left over food and drink
- Removal of all decorations & tape (no nails, tacks or tape that is likely to damage walls may be used)
- Removal of all trash to the outside dumpster (located in the front parking area of Epicenter)
- If Concession Stand is to be open, please keep in mind that all items are "marked for sale" and the coordinator will be responsible for collecting all monies.
